Online forms are used by UMassOnline to send notifications, receive input form the community, event invites, polls, surveys, questionnaires, contact forms, room management, order forms and several other items which forms are needed for.

Background


UMassOnline originally realized the need to collect data via forms when the Speaker Series events first began. Once online forms began being used for event registration, it was realized that the forms could be used for other items such as signing up to a mailing list, submitting a technical support ticket, or registering for use of a service like Confluence or JIRA. 

Utilization


Online forms are currently administered by the UMassOnline team (license holder).

Licensing


UMassOnline currently holds the license to the online form generation software and is only for use within the team. However, if you require a form, please contact us and we can assist you in building it out.
