  1. Click the Course Tools dropdown menu located in the Course Management section of the Control Panel on the left side of the screen.

  2. Click Tests, Surveys, and Pools.

  3. Click Surveys.

  4. Click Build Survey

  5. Enter the survey Name, a Description, and any Instructions.

  6. Click Submit.

  7. Clicking Submit will direct you to the Survey Canvas page.  Click Question Settings on the right side of the page.

  8. On the Survey Question Settings page, check the boxes for your settings.

  9. Click Submit

  10. Clicking Submit will direct you to the Survey Canvas page.  Hover over Create Question and click the question type you want to add.  Fill out the question form.

  11. Click Submit.  Repeat this process for all survey questions.
